Most longevity assessments don't include bone density testing. You go through bloodwork, maybe movement analysis, get some protocols optimised, and call it done.
But if you're building for longevity, not just health; bone data matters. The question isn't whether DEXA is useful in isolation. It's whether it's useful for you, given your age, risk profile, and current protocol. This post walks through exactly how to know.
The Case for Optional Bone Density Testing
Bone loss isn't dramatic. It doesn't announce itself like a lipid panel or fasting glucose. You can lose 1–2% of bone density per year in perimenopause and feel completely fine until you're 65 and a minor fall causes a fracture.
Here's the longevity medicine angle: by that point, it's too late to build bone. You can only slow further loss. If you'd caught it at 45, you'd have spent 20 years optimising density and arriving at 65 with reserve instead of deficit.
DEXA gives you that lead time. But only if you actually need it. There's no value in scanning someone with zero risk factors and excellent nutritional markers.
Who Benefits From a DEXA Scan (And Who Probably Doesn't)
Strong candidates for DEXA:
Peri-menopausal or menopausal women (bone loss accelerates sharply here, a baseline before or during transition is clinically useful)
Men over 50 with family history of osteoporosis
Anyone with past or current corticosteroid use
History of disordered eating, restrictive diets, or poor calcium/magnesium status
High-volume training (especially endurance) without corresponding strength work
Known vitamin D deficiency that hasn't fully resolved
Previous fractures or bone-related injury
Probably unnecessary:
Under 40 with no risk factors and solid nutrition
Strong training history that includes regular resistance work
Consistently optimised micronutrient markers
No family history of early bone loss
The honest clinical position: DEXA is a data point for people with risk signals, not a screening tool for everyone.
What DEXA Actually Tells You
A DEXA scan measures bone mineral density at your hip, spine, and forearm using low-dose X-rays. You get a T-score and Z-score but here's where longevity medicine differs from traditional interpretation.
Conventional medicine says: T-score below –2.5 = osteoporosis diagnosis.
Longevity medicine says: Is your bone density where it should be for you at your current age? If not, why? And what can we change?
The "why" is usually one of these:
Nutritional gap. You think you're getting enough calcium and magnesium, but something's off (absorption issue, hidden dietary restriction, or outdated assumptions about requirements).
Movement pattern. Your skeleton isn't loaded enough to signal bone formation. High cardio, low resistance. Or desk-heavy lifestyle.
Hormone asymmetry. Oestrogen, testosterone, and thyroid directly influence bone. Suboptimal bone density often points here first.
Inflammatory load. Chronic inflammation drives bone loss. DEXA can reveal this before systemic inflammatory markers alone catch it.
A single DEXA result without context is just a number. In a full assessment, where we see your bloodwork, movement history, and nutritional patterns - it becomes actionable.
The Real Value: Baseline + Reassessment
Here's where DEXA becomes genuinely useful: as a before-and-after measure.
One scan tells you where you are today. That's useful. A second scan 2–3 years later, after you've deliberately shifted your protocol, tells you whether your interventions are working. Did the targeted supplementation improve density? Did adding strength training make a difference? Is hormone optimisation showing up in your bone data?
Most longevity programmes track this through bloodwork and subjective measures. DEXA is the structural measurement that proves your skeleton is actually responding to what you're doing.
When DEXA Won't Change Anything
Be honest with yourself: do you actually need this scan?
If your protocol is already locked in and you're not planning major changes to nutrition or training, a DEXA scan might just be data that doesn't change behaviour. Longevity medicine works best when every piece of information leads somewhere. If you're not ready to act on what DEXA tells you, it's probably not the right time. But if you're building your protocol, or if you're entering a life stage (menopause, training cycle shift, dietary change) where bone health is in flux, then bone density data becomes legitimately useful.
Building Your Longevity Baseline
If you're past 40, peri-menopausal, or have risk factors, consider DEXA as part of your longevity data set. Not mandatory. Not routine. But useful if you want visibility into how your skeleton is actually responding to how you live.
